GLP-1 Titration Guide: Why Semaglutide & Tirzepatide Start Slow
The single most common GLP-1 mistake is jumping to a full dose too fast. Here's the titration math, the science behind it, and how to plan vials per phase.
GLP-1 receptor agonists like Semaglutide and Tirzepatide produce the most dramatic weight-loss data in modern pharmacotherapy. They also produce the most dramatic side-effect data — nausea, vomiting, diarrhea, reflux — when titrated too aggressively.
This is the titration logic researchers and clinicians follow.
Why titration matters
GLP-1 agonists work in part by slowing gastric emptying. The slowdown is dose-dependent. Start at a full dose, and the first meal sits in your stomach for hours, producing reflux, nausea, and frequently vomiting. Start low and ramp, and your gut adapts.
In SURMOUNT-1 and STEP 1 trials, ~5–10% of participants discontinued treatment due to GI side effects. Almost all of those events happened during dose escalation, and the most aggressive titration schedules produced the most dropouts.
Semaglutide titration
The standard clinical schedule:

That's 16 weeks to reach a full dose. Yes, really. The titration is built into the FDA-approved protocol because faster ramps produced unacceptable dropout in the trials.
For research purposes, the conservative interpretation is to follow the same schedule.
Tirzepatide titration
Slightly longer and slower:

20 weeks to top dose. The phase III trials used this exact schedule; deviating from it correlates with worse tolerability.
Vial planning
For a 5 mg Semaglutide vial reconstituted with 2 mL bacteriostatic water (concentration: 2,500 mcg/mL):
- Week 1–4 at 0.25 mg/week: 0.10 mL = 10 units per weekly dose. One vial covers ~20 weeks.
- Week 9–12 at 1.0 mg/week: 0.40 mL = 40 units per weekly dose. One vial covers ~5 weeks.
- Week 17+ at 2.4 mg/week: 0.96 mL = 96 units per weekly dose. One vial covers ~2 weeks.

What to do if side effects hit
- Reduce, don't quit. Drop back to the previous tolerated dose for 2–4 weeks before re-escalating.
- Smaller meals, slower. Eat half your previous portion and stop when satisfied, not full.
- Hydrate. GLP-1 agonists blunt thirst signaling too — set a water reminder.
- Anti-nausea support. Ginger, vitamin B6, and (with appropriate guidance) ondansetron are common tools in the literature.
Common mistakes
- Splitting weekly doses into daily ones. GLP-1 agonists are designed for weekly administration. Daily dosing produces unstable trough levels and worse tolerability.
- Skipping titration steps. "I'll just jump from 0.5 to 1.7" is the single most common cause of dropouts.
- Stopping cold turkey at goal weight. The phase III data is clear: most weight returns within a year of discontinuation. Maintenance dosing or structured tapers are the norm.

GLP-1 research protocols sometimes include compounds to mitigate side effects or preserve lean mass:
